Complete every item required on the document Claim Statement of Employee.

Make sure that you fill up the particular document and provide the most accurate information as much as possible. Keep in mind that any omission or error may cause major problems in your claim such as payment delays.

Secure a Doctors Certification document.

This is very important. Ask your physician for this document. Make sure that the document is completed and signed by your doctor. The DE 2501s Doctors Certificate portion can be signed by a chiropractor, optometrist, dentist, surgeon or physician holding a D.O. or M.D. degree, psychologist, podiatrist, licensed nurse or midwife, accredited religious practitioner, nurse practitioner, county hospital registrar, US government medical officer, and local or state health officer. Take note, however, that those who are getting temporary workers benefits need not file a Doctors Certification form.

Ask for the assistance of an agent.

If due to a disability you are deemed incapable of securing and completing a Doctors Certification form, you can ask for the assistance of an authorized agent. However, appropriate documents will be requested for this particular situation.

Mail your claim at the right time.

After completing all the necessary papers, you must file your claim to the nearest DI office in your location. The filing should not be earlier than nine working days but not later, however, than 49 working days. The documents that you need to file are the duly signed Doctors Certificate, the Claim Statement of Employee, and other supporting documents that the disability insurance California may require.
